Overview
Ziabex-M 50/1000mg Tablets are an oral antidiabetic medicine containing two complementary active ingredients: vildagliptin 50mg and metformin hydrochloride 1000mg. The combination is used for adults with type 2 diabetes mellitus when diet, exercise and single-agent therapy do not provide adequate blood glucose control, or when a clinician decides that dual therapy is appropriate. Each retail pack contains 14 tablets, commonly supplied as two strips of seven tablets.
This product is intended to support long-term glycaemic control as part of a complete diabetes plan that also includes meal planning, physical activity, glucose monitoring and regular medical follow-up. It is not an insulin substitute and is not used for type 1 diabetes or diabetic ketoacidosis.
Benefits
- Supports fasting and post-meal blood glucose control in type 2 diabetes
- Combines two complementary glucose-lowering mechanisms
- May reduce hepatic glucose production and improve insulin sensitivity
- May enhance glucose-dependent insulin response through DPP-4 inhibition
Key Features
- Vildagliptin 50mg plus Metformin HCl 1000mg fixed-dose combination
- Oral tablet dosage form
- Pack of 14 tablets
- DPP-4 inhibitor and biguanide antidiabetic combination
- Used as part of diet, exercise and blood glucose monitoring plan
Dosage and How to Use
- How to use: Swallow the tablet whole with a glass of water. Do not crush, chew or split unless your doctor or pharmacist specifically advises it. Continue diet, exercise and glucose monitoring as recommended.
- Administration: Oral administration.
- Recommended dose: Use exactly as prescribed. A commonly referenced adult regimen for vildagliptin/metformin 50/1000mg combinations is one tablet in the morning and one tablet in the evening with or just after food, but the patient-specific dose must be decided by a doctor.
- Adult dose: Patient-specific. Follow the prescriber’s instructions; many vildagliptin/metformin 50/1000mg regimens use one tablet twice daily with meals when suitable.
- Pediatric use: Not recommended for children and adolescents under 18 years unless specifically advised by a specialist.
- Elderly use: Use with caution in older adults. Kidney function should be checked before and during treatment, as metformin-related lactic acidosis risk increases when renal function is reduced.
- When to take: Usually taken with meals or immediately after meals to reduce gastrointestinal side effects associated with metformin. Follow the timing prescribed by your doctor.
- With food: Take with food or just after food unless your doctor advises otherwise.
- Maximum daily dose: Do not exceed the dose prescribed by your doctor. The maximum appropriate daily dose depends on kidney function, previous therapy and clinical response.
- Missed dose: Take the missed dose with your next meal unless it is almost time for the next scheduled dose. Do not take a double dose to make up for a missed tablet.
- Overdose: Seek medical advice or emergency care immediately if too many tablets are taken. Take the medicine pack with you. Overdose may require monitoring for lactic acidosis, hypoglycaemia and gastrointestinal symptoms.

When not to use
Allergy to vildagliptin, metformin or tablet ingredients | Diabetic ketoacidosis | Severely reduced kidney function | Severe infection or serious dehydration | Serious heart, circulation or breathing problems associated with tissue hypoxia | Severe liver disease | Acute alcohol intoxication or alcoholism | Pregnancy or breastfeeding unless a doctor specifically decides otherwise
Special populations
- Pregnancy: Do not use during pregnancy unless a doctor specifically decides that the benefit outweighs the risk. Consult a healthcare professional if pregnant, planning pregnancy or pregnancy is suspected.
- Breastfeeding: Use during breastfeeding is generally not recommended unless a healthcare professional specifically advises otherwise.
- Fertility: No product-specific fertility advice is provided in the input. Discuss fertility concerns and pregnancy planning with a doctor before use.
- Liver: Use is not recommended in severe liver disease. Liver function may need testing before and during treatment; seek advice for yellowing of the skin or eyes, dark urine, severe nausea or unexplained loss of appetite.
- Kidney: Kidney function should be assessed before and during treatment. Reduced kidney function increases the risk of metformin accumulation and lactic acidosis.
- Heart: Use caution in patients with recent heart attack, heart failure, serious circulation problems or breathing difficulty. Seek medical advice before use.
- Driving: If you feel dizzy or experience symptoms of low blood glucose, do not drive or operate machinery until you feel well and glucose is controlled.
- Alcohol: Avoid excessive alcohol intake because it can increase the risk of lactic acidosis with metformin-containing medicines.
